Long Island Elder Care LCSW, PLLC was founded in April, 2006 with a mission to provide homebound counseling to the elderly and disabled.  We felt it was an area that was under-serviced and in need of services.  The company has since expanded our program to include Case Management to assist our clients and their families to set up services needed to maintain them in the community.  Long Island Elder Care is committed to working with out of state families to set up services for their parents locally. Consultations are arranged to evaluate for Community and Chronic Care Medicaid.  Consultations for Elder Care Attorneys are arranged to evaluate and implement services at home. We have Licensed Certified Social Workers that work for Long Island Elder Care to provide compassionate quality care to our clients

The Positive Connection is a subsidiary to Long Island Elder Care providing marital, group, and individual counseling at our 63 Middle Country Rd, Middle Island location.  We service all ages from children to adults.  We are starting group sessions during the summer 2008 for children ages 11-18 to develop social skills in our 8 week program.  We also have Anxiety and Depression groups starting in the Fall of 2008. This will be a 12 week educational and therapeutic program.
