We provide consultations for elder care planning in the privacy of your loved ones home. Assessments can be done in Nassau and Suffolk Counties at a mutually convenient time. We assess for cognitive functioning, safety,emotional status and whether your loved one is eligible for Community services and Entitlements. We have over 20 years of experience in working with the elderly and disabled. We are knowledgeable about Community Resources and are experts in Community and Chronic Care Medicaid. We can help with an interim plan while waiting for entitlements. This provides reassurance that is invaluable for families and gives direction for Elder Care planning.
